On or about September 7, 2022, at jobsite located in 175 W. Olive St., Elmhurst, IL. 60126. Employees engaged in residential roofing operations were exposed to struck-by hazards while operating pneumatic nail guns without eye protection. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system: On or about September 7, 2022, at jobsite located in 175 W. Olive St., Elmhurst, IL. 60126, employees were exposed to fall hazards in excess of 18 feet while engaged in residential roofing activities. Fall protection was not being utilized adequately while performing roofing activities. Employees were wearing harness and not tied-off to a lifeline lanyard. No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On or about September 7, 2022, at jobsite located in 175 W. Olive St., Elmhurst, IL. 60126, the portable ladder used to access the roof level did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to fall hazard. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
