1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.      On or before 01/26/2015, the employer did not ensure that various equipment or machinery used in the production area at the site was effectively guarded.  The unguarded equipment includes:     a.    Eight (8) multi-spindle screw machines, actuated by a push button, had rotating parts which were unguarded;     b.   The rotating bit/chuck of (4) chamfer motors actuated by switch, did not have guarding;    c.    The Do All milling machine with retracting table was not guarded.

The metal junction box, located in the machine shop- production area was not installed in a neat workmanlike       manner.  The  box was not mounted to a fixed surface nor connected to the conduit.     b. The switch used for turning on and off chamfer motors, was a standard household light switch, and is not designed       to prevent unintentional restarting of the machinery and/or constructed and mounted to minimize inadvertent       operation.
