29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about December 8, 2016, the employer failed to ensure that effective machine guarding was in place and functioned on the Warner & Swasey horizontal CNC machine (R-1285). The equipment was in operation with the interlock device not functional. Employees were able to open the door while the machine was in operation. The Superior Quality Machine was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number 1021298, citation number 2, item number 5 and was affirmed as a final order on April 16, 2015, with respect to a workplace located at 10500 Industrial Drive, Garrettsville, OH.
