29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip point on the take up rollers at the tail of the incline conveyor and the drive rollers at the head of the incline conveyor; a) On or about 1/23/15, in the Cleaning Pit; employer failed to protect employees from the hazards created by the ingoing nip point of the take up rollers of incline conveyor #2036 resulting in an exposure to a "caught in hazard." b) On or about 1/23/15 in the Cleaning Pit; employer failed to protect employees from the hazards created by the ingoing nip point at the driver rollers of incline conveyor #2036 resulting in an exposure to a "caught in hazard." ABATEMENT DOCUMENTATION REQUIRED
