29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers was not provided, used, and/or maintained:     On December 15, 2014, the employer did not ensure employees were using heat resistant gloves during a die change out. An employee sustained 2nd degree burns to the left and right fingers when molten plastic dripped onto his hand. The heat resistant gloves would have protected him against the burns.
