29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:        On or about January 28, 2015 the employer failed to establish a program for the control of potentially hazardous energies when employees were engaged in maintenance and set up procedures of equipment within the facility. Hazardous energies include but are not limited hydraulic, pneumatic, electrical, mechanical, and thermo. Equipment includes but is not limited to hoppers, coilers, hot melters, conveyors, and augers.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about January 28, 2015, the employer failed to ensure that the ingoing nip point created by the conveyor belt and drive roller was effectively guarded on the exit side of Line 1 hot melt production line.    b) On or about January 28, 2015, the employer failed to ensure that the ingoing nip point created by the conveyor belt and drive roller was effectively guarded on the exit side of Line 3 hot melt production line.    c) On or about February 5, 2015, the employer failed to ensure that the ingoing nip point created by the conveyor belt and drive roller was effectively guarded on the exit side of Line 2 hot melt production line.

The shaft ends did not present a smooth edge.      b) On or about January 28, 2015, the employer failed to ensure that the projecting shaft ends of the Line 3 product coiler equipment were guarded to prevent employee contact. The shaft ends did not present a smooth edge.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    a) On or about January 28, 2015, the employer failed to ensure that effective guarding was in place over the chain and sprocket drive system of the Line 2 coiling equipment motor. A metal grating basket was welded onto the equipment to cover the chain and sprocket system, however the openings were too large to prevent employee contact.    b) On or about January 28, 2015, the employer failed to ensure that effective guarding was in place over the chain and sprocket drive system of the Line 2 coiling equipment motor. There was no guarding present on the equipment.

Live wiring was routed through a conduit and to a junction box which was not secured as well as wiring not encased entirely in conduit.
