29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) On or around March 20, 2015, the worker performing High Velocity Oxygen Fuel (HVOF) spray applications in the back room had facial hair that came between the sealing surface of the respirator facepiece and the face.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Cobalt listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 0.1 mg/m3: (a) The employee operating the HVOF spray application on or around March 20, 2015, in the back room was exposed to Cobalt dust or fume at an 8-hour time weighted average of 0.172 mg/m3, approximately 1.72 times the limit of 0.1 mg/m3. This limit was established to prevent respiratory hypersensitivity and asthma. Exposure calculations included a zero increment for the 130 minutes not sampled. Racine Flame Spray, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 951569, citation number 1, item 3a and was affirmed as a final order on April 29, 2014, with respect to a workplace located at 1810 William Street, Racine, WI.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): (a) The employer did not utilize feasible engineering or administrative controls to prevent employee over exposure to Cobalt dust and fume. The employee operating the HVOF spray application on or around March 20, 2015, in the backroom was exposed to Cobalt dust or fume at an 8-hour time weighted average of 0.172 mg/m3, approximately 1.72 times the limit of 0.1 mg/m3. This limit was established to prevent respiratory hypersensitivity and asthma. Exposure calculations included a zero increment for the 130 minutes not sampled.
