29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about January 16, 2015 the employer failed to protect the employees from a fall hazard of approximately 25 feet by using a personal fall arrest system. (a) The employer, Absolute Roofing & Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(11), which was contained in OSHA inspection number 98889, Citation 2 item 2, and was affirmed as a final order on September 23, 2013, with respect to a workplace located at 4470 Ridge Rd. Brooklyn Ohio 44144. (b) The employer, Absolute Roofing & Construction Inc. was also previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(11), which was contained in OSHA inspection number 769382 citation 1 item 1, and was affirmed as a final order on March 31, 2014, with respect to a workplace located at 3429 Lee Rd. Shaker Heights Ohio 44120.
