29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about June 5, 2020, the employer did not ensure that employees were protected from falling approximately 37.5 feet to the section of roof below while working on a section of roof located at 1007 Superior Avenue, Cleveland, OH 44114.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   On or about June 5, 2020, the employer did not ensure that a ladder utilized to access a  roof located at 1007 Superior Avenue, Cleveland, OH 44114 was utilized properly in that it was not extended 3 feet above the upper landing surface and did not have a grasping device.
