29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:  Maintenance Shop:  On, or about, January 29, 2015, the following drums of flammable liquids had not been provided with self-closing valves:  A.  A pink 55 gallon drum that contained diesel fuel, a Category 3 flammable liquid. B.  A 55 gallon drum that contained Brakleen, a Category 2 flammable liquid. C.  A blue 55 gallon drum that contained Camco windshield washer fluid, a Category 3 flammable liquid.

29 CFR 1910.169(a)(2)(i): Air receivers were not constructed in accordance with the 1968 edition of the A.S.M.E. Boiler and Pressure Vessel Code, Section VIII, which is incorporated by reference as specified in 1910.6:   Maintenance Shop:  On, or about, January 29, 2015, the air tank provided on the Balcrank Model 1200-003 Lube Caddy that was used to pressurize fuel lines after changing filters was not manufactured in accordance with the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:      Maintenance Shop:  On, or about, January 29, 2015, flexible cords were used in lieu of permanent wiring in the following locations:    A.  Far Wall of Shop:  A blue extension cord was used to energize a black relocatable power tap that in turn energized several battery chargers and a second relocatable power tap.    B.  Base of Mezzanine Stairs:  An orange extension cord was used to energize a series of power taps and equipment used at a large tool box.    A. Duie Pyle, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 316007822, Citation Number 1, Item Number 2 and was affirmed as a Final Order on March 23, 2012, with respect to a workplace located at Southington, Connecticut.

Maintenance Shop:  A pink 55 gallon drum that contained diesel fuel, a Category 3 flammable liquid.  B.  Wash Bay:  A tote that contained TerraCair Ultrapure Diesel Exhaust Fluid, an irritant.
