29 CFR 1910.23(c)(1): A standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder was not provided on every open-sided floor or platform 4 feet or more above adjacent floor or ground level. The railing shall be provided with a toeboard wherever, beneath the open sides: On or about 01/28/2015, dock areas 5,28, and 31 were not provided standard railings, exposing employees to an approximately 4' fall hazard.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located, and identified so that they were readily accessible without subjecting the employees to injuries. 1. On or about 1/28/2015, at Dock 5, a clear space was not kept around the ABC portable fire extingisher as a result of 1 55 gallon trash can and miscellaneous boards. 2. On or about 1/28/2015, at C1 Pole, a clear space was not kept around the ABC portable fire extinguisher as a result of several 1 gallon jugs and a stack of folded cardboard boxes. 3. On or about 1/28/2015, at Section D Pole, Row 4 & 5, a ABC portable fire extinguisher was not properly mounted.
