All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).    In Building 17, Plant 1, Door Lamination, a Porter employee cleaning the rollers and oven of the Honeycomb Machine each night was exposed to a caught inbetween hazard by not isolating the pneumatic energy control sources prior to the cleaning of the poly roller and honeycomb roller.

Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.  In Building 17, Plant 1, Door Lamination, a Porter employee who cleaned the rollers and oven of the Honeycomb Machine each night was exposed to a caught inbetween hazard by not verifiying the isolation of energy control sources prior to the start of work.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. 29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.    a.  In Building 17, Plant 1, Bead & Trim, a power press operator was exposed to an amputation hazard on PP1,a full revolution mechanical power press, due to the opening in the front of the fixed guard,8 3/8"x 2 3/4", exceeding Table O-10 requirements.    b.  In Building 17, Plant 1,  Door Fabrication, a power press operator was exposed to an amputation hazard on PP8, a full revolution mechanical power press machine due to the opening on the fixed guard, 6"x4", exceeding Table O-10 requirements.    c.  In Building 110, Plant 2, a power press operator was exposed to an amputation hazard on PP36, a L & J full revolution mechanical power press machine, due to the opening on the side of the hairpen guard 5"x6", exceeding Table O-10 requirements.

Note to paragraph (b)(8)(i) of this section: Wooden plugs driven into holes in masonry, concrete, plaster, or similar materials are not considered secure means of fastening electric equipment.    In Building 17, Plant 1, Door Lamination, at the Honeycomb Machine, an operator and porter employee was exposed to an electric shock hazard from a two receptacle electrical outlet, 110 Volts AC, that was on the floor inside the saw room and subjected to damage.
