29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area:  On or about 12/27/2018, in the Pack-out Area, employees were not safely positioned prior to re-energizing the transfer conveyor in that one employee was able to reach the moving conveyor and another employee jumped over the moving conveyor while operating the conveyor to determine the position of a roller.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):  On or about 12/27/2018, in the Pack-out Area, employees were not removed from the transfer conveyor after lockout devices were removed and the machine re-energized to determine the locations of seized rollers.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:   a) Located in Line 1A-S4, lockout devices were not affixed to each energy isolating devices of the Salvagnini S4 by authorized employees during certain servicing and maintenance activities including removing scrap, checking for leaks, and adjusting a reference bar.  b) Located in the Pack-out Area, each authorized employee did not affix a lockout device to the energy isolating device of the transfer conveyor while performing roller replacement work.   Schlage Lock Company, LLC  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1021713 , citation number 1, item number 2 and was affirmed as a final order on July, 27, 2015 with respect to a workplace located at 9017 Blue Ash Rd in Cincinnati, OH.
