29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: Employees exposed to fall hazards when required to work on top of generator enclosures were not provided with adequate personal fall arrest equipment. Employees were exposed to falls of approximately ten feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Plant- The employer failed to provide training and evaluation to operators to ensure that they were competent to safely operate forklift trucks in the workplace. Employees working on or near forklift trucks were exposed to injury from crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
