29 CFR  1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles to protect them from the rays when noncombustible or flameproof screens or shields were not used:  a.  10601 Second Street, Machesney Park, IL; Panel Welding, welding area:  On or about December 8, 2023, employees were not utilizing welding screens in the welding area to protect employees adjacent to the welding areas and others walking through the facility.  Employees were exposed to welding arc rays without appropriate protections in place.    b.  10601 Second Street, Machesney Park, IL;GenSet;  Assembly area, Bay 1-4:  On or about December 21, 2023, employees were not utilizing welding screens in Bays 1-4 to protect employees adjacent to the welding areas and others walking through the facility.  Employees were exposed to welding arc rays arc without appropriate protections in place.  Abatement certification is required of this item in accordance with 29 CFR 1910.1903(c).

29 CFR  1910.134(e)(1): General. The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.  a.  Genset:  Panel Welding, welding area:  On or about December 8, 2023, employees were exposed to respiratory stressors associated with being allowed to wear 3M Powered Air Purifying Respirator.  Employees are wearing the PAPR to protect from exposure to welding fumes when welding on galvanized steel.  The employees were not medically evaluated to determine the employee's ability to use a respirator before the employee is fit tested, or required to use the respirator in the workplace.    b.  Genset:  Assembly Area, generators:  On or about March 6, 2024, employees were exposed to respiratory stressors associated with being required to wear a 3M full face respirator 6000 series.  Employees are required to wear the respirator to protect from exposure to both Part A Multi-Thane 330 Jet Black Paint and Part B Multi thane 330/340 activator while painting the floor of the generators.  The employees were not medically evaluated to determine the employee's ability to use a respirator before the employee is fit tested, or required to use the respirator in the workplace.  This item has been corrected.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  GenSet Building, Laser 2:  On or about December 8, 2023, employees were exposed to amputation, pinching and struck by hazards, due to the table movement, while removing metal scrap from the BySprint Fiber 4020 shuttle table system and while transferring the metal sheet to the  BySprint Fiber 4020 shuttle table system. An existing light curtain, used as machine guarding, is not effective because employees can position themselves between the light curtain and the table.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  On or about December 8, 2023 through March 6, 2024, employees are exposed to respiratory hazards.  Employees  were not aware of the hazards including but not limited to Multi-Thane 300 High Solids Acrylic Polyurethane Jet Black (Pt A), Multi-Thane 330/340 Acrylic polyurethane Cure (Pt B), and R7K119 Lacquer Thinner. Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1026(d)(2)(i): The employer shall perform initial monitoring to determine the 8-hour TWA exposure for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area. Where an employer does representative sampling instead of sampling all employees in order to meet this requirement, the employer shall sample the employee(s) expected to have the highest chromium (VI) exposures.  On or about December 8, 2024, employees conducting welding operations of galvanized steel were not evaluated for initial exposure to hexavalent chromium.  The employer did not perform initial monitoring to determine the 8 hour TWA exposure for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area.  Where an employer does representative sampling instead of sampling all employees in order to meet this requirement, the employer shall sample the employee(s) expected to have the highest chromium (VI) exposures.  This item was corrected during inspection.
