The rope grab was positioned on the lifeline in such a manner that the employee would free fall more than 6 feet (1.8 m) to the unfinished ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: An employee donned with a body harness while working on a steep residential roof was exposed to 18 feet fall from height elevation hazard and had not been trained to recognize the hazards associated with the use of the equipment.
