29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about November 30, 2016 and times prior thereto, at the job site, the employer did not ensure there were guardrails at the working level of the platform to prevent fall from height hazards of greater than 14 feet to unfinished ground below.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about November 30, 2016 and times prior thereto, at the job site, an employee walking and working from a 2 inch thick by 16 inch wide by 12 foot long wood platform while installing eave blocking on the exterior of the wood framing was exposed to greater than 10 feet fall hazards to the unfinished ground below.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. On or about November 30, 2016 and times prior thereto, employees were installing studs and raising and bracing interior walls less than a foot away from unguarded window openings which were greater than 6 feet (1.8 m) above the finished ground below.
