29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (a) For employees operating bread making machines and during the construction of installing the new Flower Reclaiming System at 6490 Parkland Drive, Sarasota, FL 34243, as noted on or about 02/04/2015, employees were offered the Moldex, 2700N95, Particulate Respirator, without having been provided the Appendix D information.
