29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) South exposure: On or about 11/21/2014, workers removing existing gutters from a house were not provided with fall protection and exposed to a 14 foot fall from the unguarded platform.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a)South Exposure: On or about 11/21/2014, a worker performing miscellaneous roof repairs on a terra cotta tile roof was not provided with fall protection and exposed to a 20 foot fall from the roof. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
