29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Front of Row House - An employee was observed climbing a portable metal extension ladder to the roof and the ladder side rails were located below the roof landing and a grasping device was not provided, exposing the employee to a 26 ft. fall hazard, on or about 04/23/2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof, Irving Street Side - An employee was observed standing at the edge of a flat roof section while installing slate shingle on a turret section without fall protection and exposed to a 26 ft., fall hazard, on or about 04/23/2015. Top Crew LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1022048, citation number 1, item number 2, and was affirmed as a final order on April 07, 2015, with respect to a workplace located at 5545 45th Street, NW Washington DC 20011.
