Employees were exposed to struck-by and caught-in-between hazards.    B)     Nolan Logistics, Inc., located at 1250 West Laskey Road, Toledo, OH 43612: On or about January 29, 2015, the employer did not remove the yard truck # 2 Ottawa Kalmar (Model 4x2, Serial # 328839) from use based on mechanical defect such as, but not limited to, broken steering wheel and defective starter solenoid. Employees were exposed to electrical hazards, as well as struck-by and caught-in-between hazards.    Nolan Logistics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.178(p)(1)), which was contained in OSHA Inspection # 982180, Citation number 1, Item number 2a, and was affirmed as a final order on August 21, 2014, with respect to a workplace located at 1250 West Laskey Road, Toledo, OH 43612.

Employees were exposed to struck-by and caught-in-between hazards.
