29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:      (a) On or about January 27th, 2015, at a job site located at 18011 E. Bass Pro Drive in Independence, MO, an employee was exposed to the hazard of falling more than 6 feet to the ground while working on a low-slope roof in that no fall protection system was in place or in use at the time of the inspection.    The Godard and Son Roofing Co., Inc., was previously cited three times for a violation of this occupational safety and health standard.  The first citation was contained in OSHA inspection number 314469834, citation number 01-001, and was affirmed as a final order on 2/9/2012 with respect to a workplace located at 3805 Frederick Ave. in St. Joseph, MO.  The second citation was contained in OSHA inspection number 1003216, citation number 01-001, and was affirmed as a final order on 12/11/2014 with respect to a workplace located at 1130 Emanuel Cleaver Blvd. in Kansas City, MO.  The last, and most recent citation was contained in OSHA inspection number 1009805, citation number 01-002, and was affirmed as a final order on 12/30/2014 with respect to a workplace located at 5151 NW 85th Street, Kansas City, MO.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      (a) On or about January 27, 2015, at a job sited located at 18011 E. Bass Pro Drive in Independence, MO, employees performing roofing work were exposed to fall hazards while using a portable ladder to gain access to an upper roof level from a lower roof level.  The ladder's side rails did not extend the required 3 feet above the upper landing surface.      The Godard and Son Roofing Co., Inc., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 314469834, citation number 01, item number 003, and was affirmed as a final order on February 9, 2012, with respect to a workplace located at 3805 Frederick Ave. in St. Joseph, MO. 29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   (a) On or about January 27, 2015, at a job site located at 18011 E. Bass Pro Drive in Independence, MO, employees performing roofing work were exposed to fall hazards while using a portable ladder to access an upper roof level from a lower roof level.  The subject A-frame type step ladder was incorrectly being used in its collapsed position as a straight ladder.   The Godard and Son Roofing Co., Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1926.1053(a)(8)], which was contained in OSHA inspection number 314469834, citation number 01, item number 002, and was affirmed as a final order on February 9, 2012, with respect to a workplace located at 3805 Frederick Ave. in St. Joseph, MO.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
