29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by pinch points and moving parts: A.) Arvite Technologies, Inc., 2731 West 11th Street, Erie PA 16505, on or about January 29, 2015 and times prior thereto: Employees operate a Wysong shearing machine (M/N: H-52, S/N: H52-504) to perform activities including, but not limited to, cutting cardboard strips. The shearing machine's hold down was adjusted / set to high off the table to function properly as a guard and was not adequately guarded to prevent the entry of hands / fingers into the pinch point between the hold down and the table.

Arvite Technologies, Inc., 2731 West 11th Street, Erie PA 16505, on or about January 29, 2015 and times prior thereto: Employees operate a Wysong shearing machine (M/N: H-52, S/N: H52-504) to perform activities including, but not limited to, cutting cardboard strips. The shearing machine was not equipped with an adequate guard to prevent the entry of hands/fingers into the point of operation. The shearing machine's hold down did not function as an adequate guard because it was set / adjusted too high above the table.
