29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    a)  On or about 01-23-15 at W3351Garton Road in Sheboygan Falls, the employer did not provide and/or instruct an employee in adequate forklift operator safety training such as struck-by and crushing hazards prior to operating the forklift outside on slippery surface with indoor use tires.  The employer's program did not include formal instruction as required by the standard.

29 CFR 1910.178(l)(3)(i)(M): The employer did not review the operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate:  a)  At the worksite, located at W3351 Garton Road in Sheboygan Falls, the employer did not train the forklift operator on the truck application section of the Clark Operator's Manual, which stated that cushion (solid) tire trucks are designed for use inside on smooth dry surfaces.

The forklift did not receive any repair work after a 01-23-15 tip-over incident when the horn quit working.
