29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a. On February 3, 2015, near the east side exterior wall, 2x4 and 2x6 lumber, pieces of conduit, and debris were on the ground in the path to the building.

On February 3, 2015, on the east side exterior of the building, employees on a temporary platform troweling  concrete in form work were not protected from falling from an height of approximately 14 feet.

On February 3, 2015, on the east side interior of the building, there were no stairways or ladders provided for employees to access and go over the top of formwork to get to the work area on the exterior of the building.
