29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use:      a)  On February 6, 2015, on the south side of the construction site, a job-made wooden extension ladder, approximately 11 feet in length, with non-uniformly spaced rungs was used to access the next working level platform, exposing employees to fall hazards up to 9 feet.

On February 6, 2015, under the west side of the ladder access to the next working level, 2x4 lumber and pieces of plywood, and debris were on the ground in the path around to the front of the construction of the building.

On February 6, 2015, employees, laying concrete cinder blocks, were working above other employees and the temporary work platforms did not have toeboards installed to prevent objects from falling.
