Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to the hazard of being crushed: a) On or about February 2, 2015, and times prior to that date, employees were exposed to crushing hazards in the event of a roll-over/tip over created by operating the Hyster Lift Truck model # S150A, serial # A024D05576G, without adequate restraint systems being installed or used. Abatement note: Among other methods, one feasible and acceptable method to correct this hazard would be to install seat belts on the truck being operated without them and to enforce the requirement that the operators of the powered industrial truck use the seat belts.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: a) On or about February 2, 2015, and at times prior to that date, employees were exposed to "struck by" hazards from steel storage racks where the base plates on storage rack columns were not anchored to the floor. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the requirements ANSI MH 16.1:2008 which states that every column footplate of each pallet rack installation must be properly anchored to the floor.
