29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed:  a)  Maintenance Work Table:  On or about January 30, 2015, an employee was exposed to the hazards of electrical shock and arcing parts while working near the wall mounted 8"x 8" metal junction box, which had two 3/4" knockout removed.  b)  NDS Area:  On or about January 30, 2015, an employee was exposed to the hazards of electrical shock and arcing parts while working near the wall mounted 2 gang metal receptacle box, which had a 1/2" knockout removed.  c)  West Wall, near Line #5:  On or about January 30, 2015, an employee was exposed to the hazards of electrical shock and arcing parts while working near the wall mounted Circuit Breaker Panel "D", which had a 1/2" knockout removed.  d)  West Wall, near Line #5:  On or about January 30, 2015, an employee was exposed to the hazards of electrical shock and arcing parts while working near the wall mounted Circuit Breaker Panel "D", which had circuit breaker locations 2, 4, and 6 open, thus exposing the panel's bus bar.
