29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Production floor, On or about May 11, 2018, and at time prior thereto, employees were exposed to a caught-in hazard, while operating the Regulator Ball Valve Press, #1240, that did not have the press two hand-trips arranged by design and construction to require the use of both hands to cycle the press.
