29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operators and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a)NAMJet LLC at 4965 Kingston St., Denver, CO 80239:  On and before February 3rd, 2015 the employer did not ensure that one or more methods of machine guarding such as barrier guards, two-hand tripping device, electronic safety devices, etc. were provided to prevent employees from having any part of their body in the danger zone. This condition exposed the operator and other employees to injuries caused by unprotected rotating parts, point of operation, and ingoing nip points. This condition includes but is not limited to:      (1)Shibaura Horizontal Boring Mill, Model BTD-11R- Rotating screw auger presented an amputation hazard.  (2)Bullard Vertical Turret Lathe - Rotating parts hazard condition.  (3)Kingston Horizontal Lathe - Rotating parts hazard condition.  (4)American Pacemaker Horizontal Lathe" - Rotating parts hazard condition.  (5)Atlantic Shear- Point of operation hazard condition regarding the hold downs.  (6)Atlantic Hydraulic Press Brake-Unguarded point of operation presented crushing and amputation hazards.  (7)Horizontal Band Saw-Unguarded point of operation presented laceration and amputation hazards.

29 CFR 1910.215(b)(9):  Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:      (a)NAMJet LLC, at 4965 Kingston St., Denver, CO 80239:  On and before February 3rd, 2015, the employer did not ensure that the bench grinder located in the production area had the correct distance between the wheel periphery and the end of the peripheral member at the top and did not exceed ¼ inch because the adjustable tongue was missing.  This condition exposed the employees to the hazard of flying chips and debris.

29 CFR 1910.219(d)(1):  Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):      (a)NAMJet LLC, at 4965 Kingston St., Denver, CO 80239:  On and before February 3rd, 2015, the employer did not ensure that the pulley associated with the fence motor located at the rear of the Atlantic Shear was guarded, in that, the belt and pulley apparatus guard was missing. This condition exposed employees to in-going nip point hazard.

29 CFR 1910.219(e)(1)(i): Horizontal belts operated 7 feet or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o):      (a)NAMJet LLC, at 4965 Kingston St., Denver, CO 80239: On and before February 3rd, 2015 the employer did not ensure that the belt associated with the fence motor on the backside of the Atlantic Shear was guarded. This condition exposed employees to in-going nip point hazards associated with rotating pulley and belt assembly.

29 CFR 1910.242(b):  Compressed air used for cleaning purposes was not reduced to less than 30 psi:       (a)  NAMJet LLC, at 4965 Kingston St., Denver, CO 80239:  On and before February 3, 2015, the employer did not ensure the compressed air nozzles used for cleaning off equipment in the facility was reduced to less than 30 psi. The compressed air line was pressurized at approximately 95 psi. This condition exposed employees using the air nozzles to the hazard of an embolism in the blood stream.

29 CFR 1910.303(b)(1):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    (a)NAMJet LLC at 4965 Kingston St., Denver, CO 80239:  On and before February 3rd, 2015, the employer did not ensure to provide a workplace free from recognized electrical hazards, in that, the power supply cordset had exposed inner conductors. This condition exposed the employees to the hazard of electrical shock. This condition includes, but is not limited to:    (1)Plug end of an electrical power cord to a welding unit. (2)Flexible cord wiring to a drill press.
