29 CFR 1926.252(b): When debris was dropped through holes in the floor without the use of chutes, the area onto which the material(s) were dropped was not completely enclosed with barricades not less than 42 inches high and not less 6 feet back from the projected edge of the opening above: On or about January 26, 2015 at a worksite located at 3600 Geyers Chapel Road Wooster, OH, during steel erection activities including but not limited to cutting an opening in metal deck floor and removing construction debris, the employer did not ensure the area onto which material is dropped was enclosed with barricades, six feet back from the opening above, and posted with signs.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On or about January 26, 2015 at a worksite located at 3600 Geyers Chapel Road Wooster, OH, during steel erection activities including but not limited to cutting an opening in metal deck floor and removing construction debris, the employer did not ensure all employees using the elevated steel grating platform to remove debris were protected from fall hazards approximately 17 feet by using a fall protection system/device.
