29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On February 5, 2015, on the east side of the building on the second level roof, there was no fall protection for employees along the 52 foot length as the parapet was about 18 inches high. Employees working on the roof were exposed to falls from a height of about 13 feet to the first level concrete roof below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step: a. On February 5, 2015, near the north side interior of the first level, the top and top step of a 6 foot long stepladder was used to tear formwork off the wall were exposed to the hazards of falling 5 to 6 feet to the concrete floor below.
