29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a. On February 5, 2015, inside the structure, there was no fall protection for an employee on a mobile scaffold platform as the employee had to untie his lanyard from the structural supports when the scaffold was moved, exposing employees to the hazard of falling from a height of 12.75 feet to the concrete floor below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step: a. On February 5, 2015, inside the structure, the top and top step of a 12 foot long stepladder were used to access a 12.75 foot mobile scaffold platform, exposing employees to the hazard of falling from heights up to 12.75 feet.
