29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) CTA Builders, Veterans Affairs Medical Center, Richmond: On or about 02/04/15, employees on a canopy were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems, as a result employees were exposed to a 19' 4" fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
