29 CFR 1926.502(f)(2): Warning line systems did not consist of ropes, wires, or chains, and supporting stanchions erected in accordance with (f)(2)(i) through (f)(2)(v) of this section:    a)      CTA Builders; VA Medical Center Richmond; Dialysis Expansion: On or about 04/09/2015, warning lines were not erected with ropes, wires or chains in that the warning lines were erected with caution tape, thus exposing employees to a fall hazard.    No abatement certification or documentation required.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    a)      CTA Builders; VA Medical Center Richmond; Dialysis Expansion: On or about 04/09/2015, the employer did not assure that employees received the proper training in the use and installation of warning lines, thus exposing employees to a fall hazard.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever was less, were not equipped with one stair rail system along each unprotected side or edge:    a)    CTA Builders, VA Medical Center Richmond, Dialysis Expansion Building : On or about 04/09/15, employees required to access the stair tower were not protected from falling by the use of a stairrail system along the unprotected side, as a result employees were exposed to a 13' 5" fall hazard.     No abatement certification or documentation required.
