Employees in the shop were exposed to amputation and struck-by hazards in that several cut-off saws were not guarded as follows:          1. In the "owner's shop", a Ridgid cut-off saw with an abrasive 16" cutting wheel, not guarded.     2. In the "owner's shop", a Ridgid cut-off saw with an abrasive 8" cutting wheel, not guarded.     3. In the main shop, a least three Everett cut-off saws with abrasive 16" cutting wheels, not guarded.     4. In the main shop, a Ridgid cut-off saw with an abrasive 16" cutting wheel, not guarded. Employees in the metal shop were exposed to amputation and struck-by hazards while using unguarded portable angle grinders to shape and cut ornamental metal as follows:            1. In the "owner's shop", a central pneumatic portable angle grinder with 3 inch cut off wheel, not guarded.      2. In the main shop, at least three Makita portable angle grinders with 4 inch grinding wheels, not guarded.      3. In the main shop, at least two Makita portable angle grinders with 6 inch cut off wheels, not guarded.       4. In the main shop, a Milwaukee portable angle grinder with 6 inch cut off wheel, not guarded. McSwain Metal Fabrication, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 313720740, citation number 1, item number 2 and was affirmed as a final order on August, 23, 2010 , with respect to a workplace located at 2800 S. Vallejo Street, Englewood, CO 80110.
