29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) McSwain Metal Fabrication Inc. at 2800 S Vallejo St. Englewood, CO 80110: On or before 2/23/21 the employer did not ensure that the Hynd Mech horizontal bandsaw [model: S-20, serial:21093514] was guarded in accordance with this standard. Portions of the blade outside the working part of the blade were exposed. This condition exposed employees to laceration and amputation hazards. 29 CFR  1910.215(b)(5): The angular exposure of grinding wheel periphery and sides for safety guards used on surface grinding machine(s) employing the wheel periphery exceeded 150 degrees:   (a) McSwain Metal Fabrication Inc. at 2800 S Vallejo St. Englewood, CO 80110: On or before 2/23/21 the employer did not ensure that the Baldor cutoff saw was guarded in accordance with this standard. The automatically adjusting guard was pinned in the open position making the guarded circumference of the cutoff wheel 180 degrees. This condition exposed employees to flying chips and sparks as well as laceration and amputation hazards.  Abatement Note: Abatement certification and documentation are required for this item. The guard was removed. This condition exposed employees to laceration and amputation hazards as well as flying chips and sparks.
