29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) On February 2, 2015 the inspection revealed that the roll forming/bending machine was not provided with at least one of the following methods of point of operation guarding: a point of operation guard, a point of operation device, a point of operation awareness barrier (if a point of operation guard or device is not feasible), or an emergency stop device. The roll forming/bending machine was located in the middle of the shop bay at the time of the inspection.   (b) On February 2, 2015 the inspection revealed that the Cincinnati Bickford radial drill did not have a point of operation guard. The drill was located in the mill room.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half-hour.  (a) On February 2, 2015 the inspection revealed that oxygen and acetylene cylinders were stored within about 3 feet of each other and were not separated by a fire resistant barrier, potentially exposing employees to serious injury in the event of a fire. The storage cages were located in the center of the work bay at 200 Lake Drive in Hammond, Oregon.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects:   (a) On February 2, 2015 the inspection revealed that employees were potentially exposed to serious injury from electric shock because the terminals for welding leads on the Lincoln Arc Welder (flux core welder) were not protected from accidental contact. The Lincoln Arc Welder was located in the welding area of the shop at the time of the inspection.

29 CFR 1910.254(c)(1): Installation of arc welding equipment including power supply was not in accordance with the requirements of Subpart S, 29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  (a) On February 2, 2015 the inspection revealed that the outer insulation on the power cord for Lincoln Arc Welder was damaged. The Lincoln Arc Welder was located in the welding area of the shop at the time of the inspection.
