29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a)Old Dominion Freight Line Inc. at 5601 Holly Street, Denver, CO: On and before 12/8/14, Old Dominion Freight Line Inc. did not ensure that protective equipment was used when necessary whenever hazards capable of causing injury and impairment were encountered. Employees were directed by a supervisor to contain and clean up a spill of approximately 15 gallons of epoxy resin leaking from a barrel in a truck trailer. The epoxy resin was a flammable liquid. The employees did not use protective eyewear or respiratory protection. The employees performed the work initially without protective gloves. This condition exposed the employees to a chemical hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area: (a)Old Dominion Freight Line Inc. at 5601 Holly Street, Denver, CO: On and before 12/8/14, Old Dominion Freight Line Inc. did not ensure that employees were provided effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area. Employees were directed by a supervisor to contain and clean up a spill of approximately 15 gallons of epoxy resin leaking from a barrel in a truck trailer. The epoxy resin was a flammable liquid. The employees and supervisor had not received training on chemical spill response procedures. This condition exposed the employees to a chemical hazard.
