29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a.    On or about January 22, 2015 and prior thereto, maintenance employees were not required to deenergize the machine and perform lockout tagout when making adjustments to Staple Machine 104.       b.    On or about January 22, 2015 and prior thereto, machine operators  were not required to deenergize the machine and perform lockout tagout when the guard over the flywheel on Staple Machine 104 was lifted  to verify the position of the cutter head and the flywheel was manually moved.    c.    On or about January 22, 2015 and prior thereto, maintenance employees were not required to deenergize and perform lockout tagout when making adjustments to Staple Machine 113.       These conditions exposed employees to the hazard of being struck by or caught between moving parts of the staple machines.

Employees who were First Responders provided first aid where blood was present and disinfected environmental surfaces and equipment contaminated with blood.
