29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition; it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact;      On or about February 6th, 2015 at the above addressed job site, employees performing concrete plastering operation were not provided with appropriate gloves, exposing them to come in contact with wet Portland cement (Titan America Stucco Cement) that can cause skin sensitization, dermatitis, caustic burns, burns may result in blisters, dead or hardened skin, or black or green skin. In severe cases, these burns may extend to the bone and cause disfiguring scars or disability.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  On or about February 6, 2015 at the above addressed job site, the employer did not develop or implement a written hazard communication program describing hazardous chemicals, such as but not limited to; Portland cement and gasoline for employees performing concrete plastering operations.
