(b) On or about 02/05/15 - Tool & die shop, the two Bridgeport VMC 760 CNC mills had the front access door safety interlocks by-passed. (c) On or about 02/05/15 - Machine shop/Tool & Die shop, two horizontal lathes did not have chuck guards installed.
