29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about January 14, 2015, at 413 Devon Brook Lane, Lawrenceville, Georgia:  Four employees performing framing work were exposed to struck-by hazards from the falling tools and materials of the employees working above them when they did not use head protection.

On or about January 14, 2015, at 413 Devon Brook Lane, Lawrenceville, Georgia:  a) Five employees performing framing work were exposed to struck-by hazards from nails, and wood chips while operating nail guns without using eye protection.   b) Two employees performing framing work were exposed to struck-by hazards from wood chips, and sawdust while operating a Makita 5007F circular saw without using eye protection.

29 CFR 1926.304(g)(1): The sides of the lower blade were not guarded to the full diameter of the blade by a device that would automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection:  On or about January 14, 2015, at 413 Devon Brook Lane, Lawrenceville, Georgia:  Two employees performing framing work were exposed to amputation hazard when they used a Makita 5007F circular saw that had a piece of wood wedged next to the blade preventing the lower blade guard from operating.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about January 14, 2015, at 413 Devon Brook Lane, Lawrenceville, Georgia:  Two employees performing framing work were exposed to electric shock hazards while operating a Makita 5007F, 120 volt, 15 amp, circular saw that was connected to an extension cord with strain relief damage below the plug.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 14, 2015, at 413 Devon Brook Lane, Lawrenceville, Georgia:  a) Three employees performing framing work were exposed to a fall hazard of approximately 20-feet and greater when they worked inside the trusses and at the unprotected edges of the house without using fall protection.   b) Two employees performing framing work were exposed to a fall hazard of approximately 12-feet and greater when they worked at unguarded wall and window openings without using fall protection.
