29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) The safety interlocks are bypassed allowing employees to access the hazardous moving parts of the Line 21 MGS Machine (model number IPP-690D, serial number 15621) while filling the label (insert) hopper exposing the employees to amputation and entanglement hazards.    (b) The safety interlocks are bypassed allowing employees to access the hazardous moving parts of the Line 22 MGS Machine (model number IPP-590D, serial number 15395) while filling the label (insert) hopper exposing the employees to amputation and entanglement hazards.    (c) The safety interlocks are bypassed allowing employees to access the hazardous moving parts of the Line 23 MGS Machine (model number IPP-890D, serial number 15396) while filling the label (insert) hopper exposing the employees to amputation and entanglement hazards.
