OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to Struck-by/Caught between hazards associated with damage to the structural components of storage rack systems,  damaged locking mechanisms, and a lack of information conveyed to employees on the load limits associated with storage rack systems:  a.)	Warren Industries, Inc.; warehouse - On or about 3/16/22, and times prior thereto, a horizontal beam and a diagonal beam were bent and damaged.     b.)	Warren Industries, Inc.; warehouse - On or about 3/16/22 and times prior thereto, several locking mechanism associated with the racks  were damaged and were not locking the racks into place.    c.)  Warren Industries, Inc; warehouse - On or about 3/16/23 and at times prior thereto, temporary racking was missing an anchor bolt.  d.)	Warren Industries, Inc.; warehouse - On or about 3/16/22, and times prior thereto, the load ratings for the  racking systems  were not displayed and/or conveyed to employees.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  a.)	Warren Industries, Inc.; warehouse - On or about 3/16/22, and times prior thereto, the rear tire on Caterpillar propane forklift #158 was damaged.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1904.29(b)(1):29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  a.) Warren Industries, Inc. - On or about 3/16/23 and at times prior thereto, the employer did not fill out Column F on the OSHA 300 Injury and Illness log properly.  The employer only filled out the part of the body affected instead of also listing the injury and the object/substance that made the person injured or ill.   This occurred on case #1 of Calendar Year 2023 which happened on 2/22/23.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
