On or about 02/4/2015, employees working In the Core Laboratory and Cw139C at the Department of Veterans Affairs Medical Center located at 1030 Jefferson Ave in Memphis, TN were exposed to hazardous materials (eye, skin, and/or respiratory irritants) for which the employer did not compile a complete chemical list of all hazardous materials used in the work place.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1450(e)(4): The employer did not review and evaluate the effectiveness of the Chemical Hygiene Plan at least annually and update it as necessary  On or about 02/4/2015 and at times prior to, employees working at the Department of Veterans Affairs Medical Center located at 1030 Jefferson Ave in Memphis, TN, were exposed to chemical hazards and the employer had not updated (last up-dated 2012) the written Chemical Hygiene Plan annually.
