29 CFR 1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure. A. Culy Contracting, Inc. worksite located on Cable Rd., Lima, Ohio: On or about February 10, 2015, the employer did not ensure all spreader bars were installed on the North end of the trench box. The bottom spreader bar was not installed as recommended by the manufacturer.
