29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Culy Contracting, Inc. on or about April 23, 2015, at a trench located on Caroline St. east of Williams St. in Paulding, OH: The employer failed to ensure an employee was adequately protected from cave-ins while preparing a water main pipe for the installation of bypass valve in a 7 ft. deep unprotected trench. (b) Culy Contracting, Inc. on or about April 23, 2015, at a trench located on Caroline St. west of Williams St. in Paulding, OH: The employer failed to ensure employees were adequately protected from cave-ins while installing a bypass valve onto a water main pipe in a 6.6 ft. deep trench.
