29 CFR 1926.403(e): All splices and joints and free ends of conductors were not covered with insulation equivalent to that of the conductors or with an insulating device designed for the purpose: On or about February 10, 2015 at the worksite located at or near 32 Weatherstone Point, Little Rock, Arkansas, employees used a damaged electrical cord connected to a powered circular saw during walk around with supervisor on site. This condition exposed employees to electrical hazard.
