29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about March 6, 2018, an employee without a hard hat was working underneath an employee working directly above on an upper level who had tools and material which could have fallen and struck the employee. This condition exposed the employee to struck by hazards.

29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about March 6, 2018, an employee without eye protection was  exposed to hazards while using the following equipment:  Instance a) Employee on the framing using using a pneumatic nail gun.  Instance b) An employee operating a circular saw to cut boards at ground level without eye protection.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.    On or about March 6, 2018, the electrical extension cord used to power a circular saw was damaged in several places, exposing the internal insulation. The employer did not inspect electrical cords daily and allowed them to be used with visible damage. This condition exposed employees to electrical shock and burn hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   V & G Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1036207, citation number 1 item number 2 and was affirmed as a final order on March 19, 2015, with respect to a workplace located at 32 Weatherstone Point, Little Rock, Arkansas.  On or about March 6, 2018, employees were working at heights of 10 feet without fall protection while performing residential framing construction. This condition exposed employees to the hazard of falling to the ground below.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  On or about March 6, 2018, the metal extension ladder used for roof access was placed at a 45 degree angle, which greater than one-quarter of the fourteen feet working length of the ladder. This condition exposed employees to fall hazards.
